The fact everyone was quoting that really stuck with me is that the NHS in Britain is the third largest employer in the world, behind the Chinese Army (PLA) and India's rail system. So this tiny little place (compared to those two behemoth countries) requires that many people in its health service? How can that possibly be scaled up?

Because it is a bureaucracy that has had a long-time to grow, and it is a sacred cow that no one seems willing to cut down to size.
